Required Documentation

Before you make your visit to the Enrollment Staffing Unit (ESU), you will need to gather the following items, and bring them with you in order to complete the hiring process.

Completed enrollment forms with required documentation
All new teachers, administrators, school business managers, and computer technicians are required to provide official college transcripts with degree(s) listed
All candidates applying for one of the following job titles are required to submit a college transcript with 60 semester hours or an associate's degree: School Assistant, School Assistant Bilingual, School Assistant Bilingual - Spanish, School Library Assistant, Special Education Classroom Assistant, Teacher Assistant, and Teacher Assistant Montessori. All other ESP job titles need to provide a high school diploma or GED
Illinois State Teaching Certificate (if you do not have an Illinois State Teaching Certificate, please visit the ISBE website at www.isbe.net - Teachers Only)
Photo identification and work authorization documents
Social security card (providing your social security number is not sufficient)
Birth certificate
Voided check (Optional - if you wish to have your paycheck directly deposited into your checking account)
A physical examination form completed and signed by a licensed physician. If you do not currently have a physician, click below for a listing of City of Chicago Department of Health Clinics to schedule an appointment for a physical examination.
